Case Management
At present case management is not a profession in the UK,
but is recognised as a specialist practice within a profession
such as nursing, occupational therapy and social work.
Case management assists the client to reach their best
possible level of functional ability through the efficient
delivery of quality services.
One of the aims of case management is to provide a seamless
progression between rehabilitation and long term care needs
where this is required.
A case management plan includes risk assessment, making
ongoing assessments, identifying short, medium and long term
goals and establishing any required therapy, as recommended by
relevant therapists, by resourcing appropriate professionals
and co-ordinating the service across the State and Private
Sector. End of month reports are provided and reassessment
reports prepared as requested.
