HACM Limited office hours are 9am to 5pm, Monday to Friday.
All contact with Case Management Services should be between
these hours, however, in certain circumstances, where
possible, pre-booked appointments can be arranged outside
these hours.
If carers/support workers are provided by an Agency or Care
Company, a branch care coordinator will be available for
telephone consultation by means of a 24 hour on call service.
Following identification of need, and by prior agreement, HACM
will provide telephone numbers of three HACM Limited Case
Managers so that they can be called in the event of an
emergency.
