Introducing our Case Managers
SAM HARRIS
Sam is based in Nottinghamshire. She holds professional memberships with The British Association of Brain Injury Case Managers (BABICM) as an Advanced Member; Case Management Society UK (CMSUK); Royal College of Nursing and the Nursing and Midwifery Council (NMC).
Sam is a Registered General Nurse, Managing Director and Senior Case Manager of HACM Limited. She worked within the NHS for 15 years where she gained extensive experience in a variety of trauma and rehabilitative settings. Upon qualifying as a General Nurse, Sam worked within post-acute settings and specialised in Trauma and Orthopaedic roles, which included neuro and spinal rehabilitation. In 2001 Sam combined her case management role with work at the Queen’s Medical Centre, Nottingham as Practice and Professional Development Sister in the Directorate of Trauma and Orthopaedics.
Sam was previously the Chair and a Director of Case Management Society UK (CMSUK) and continues to be involved with the Standards, Research and Education committee. She is also a Trustee of Headway, Nottingham.
Sam has worked as a Case Manager since 2002 specialising in the coordination of rehabilitation of individuals with complex brain, orthopaedic and spinal injury. Sam coordinates rehabilitation programmes for adults and children with complex and multiple physical difficulties. Sam works closely with multi-disciplinary teams, developing care plans, and co-ordinating complex packages of care. She also devises care packages, including 24-hour packages, and provides a recruitment and training function for support workers employed to work with HACM’s clients.
EMMA CARTER
Emma is based in Leicestershire. She holds professional memberships with The British Association of Brain Injury Case Managers (BABICM) as an Advanced Member; Case Management Society UK (CMSUK) and the Nursing and Midwifery Council (NMC).
Emma is a Registered General Nurse with specialist training in Paediatrics, Neurology and Neuro-surgery. She has worked as a Case Manager since October 2007 and with HACM Limited since February 2008.
Emma coordinates rehabilitation programmes for adults and children who have sustained an acquired brain injury, Cerebral Palsy, undergone amputation or experienced traumatic physical and psychological injuries as a result of medical negligence or personal injury. Emma oversees multi-disciplinary teams, develops care plans and co-ordinates complex packages of care. This includes liaison with and referral to other statutory services across health and social services sectors.
Emma works closely with the families to ensure that the client’s ongoing needs are met. Her role also includes the preparation of reports, rehabilitation plans and goal setting with clients.
Our Office Team
Our Office Team provides support to HACM’s Clients, Case Managers and Care Teams.
Debbie and Melisa typically work behind the scenes to organise and coordinate office administration on a day-to-day basis, to ensure organisational effectiveness and efficiency. The Office Team are responsible for streamlining administrative procedures, managing databases, finance, and HR.
