Introducing our Case Managers

SAM HARRIS

Sam is based in Nottinghamshire. She holds professional memberships with The British Association of Brain Injury Case Managers (BABICM) as an Advanced Member; Case Management Society UK (CMSUK); Royal College of Nursing and the Nursing and Midwifery Council (NMC).

Sam is a Registered General Nurse, Managing Director and Senior Case Manager of HACM Limited. She worked within the NHS for 15 years where she gained extensive experience in a variety of trauma and rehabilitative settings. Upon qualifying as a General Nurse, Sam worked within post-acute settings and specialised in Trauma and Orthopaedic roles, which included neuro and spinal rehabilitation. In 2001 Sam combined her case management role with work at the Queen’s Medical Centre, Nottingham as Practice and Professional Development Sister in the Directorate of Trauma and Orthopaedics. 

Sam was previously the Chair and a Director of Case Management Society UK (CMSUK) and continues to be involved with the Standards, Research and Education committee.  She is also a Trustee of Headway, Nottingham.

Sam has worked as a Case Manager since 2002 specialising in the coordination of rehabilitation of individuals with complex brain, orthopaedic and spinal injury.  Sam coordinates rehabilitation programmes for adults and children with complex and multiple physical difficulties.  Sam works closely with multi-disciplinary teams, developing care plans, and co-ordinating complex packages of care. She also devises care packages, including 24-hour packages, and provides a recruitment and training function for support workers employed to work with HACM’s clients.

EMMA CARTER

Emma is based in Leicestershire. She holds professional memberships with The British Association of Brain Injury Case Managers (BABICM); Case Management Society UK (CMSUK) and the Nursing and Midwifery Council (NMC).

Emma is a Registered General Nurse with specialist training in Paediatrics, Neurology and Neuro-surgery. She has worked as a Case Manager since October 2007 and with HACM Limited since February 2008.

Emma coordinates rehabilitation programmes for adults and children who have sustained an acquired brain injury, Cerebral Palsy, undergone amputation or experienced traumatic physical and psychological injuries as a result of medical negligence or personal injury. Emma oversees multi-disciplinary teams, develops care plans and co-ordinates complex packages of care. This includes liaison with and referral to other statutory services across health and social services sectors.

Emma works closely with the families to ensure that the client’s ongoing needs are met.  Her role also includes the preparation of reports, rehabilitation plans and goal setting with clients. 

CHRIS MORAN

Chris is based in Derbyshire. He holds professional memberships as Registered Social Worker with Social Work England; The British Association of Brain Injury Case Managers (BABICM) and Case Management Society UK (CMSUK).

Chris has worked as a Case Manager since October 2016.  Chris has extensive experience of working with complex and challenging clients within a variety of specialities, including in the NHS, third sector organisations and private health and social care settings.  Prior to becoming a Case Manager, Chris worked as a Social Worker within Substance Misuse and Mental Health Services, and then within a Local Authority Children and Families’ service.  He also has a background of working with adults who have acquired brain injury or a learning disability.

Chris implements and coordinates rehabilitation programmes for adults and children who have sustained an acquired brain injury, spinal injury or experienced a traumatic physical and psychological injuries as a result of medical negligence or personal injury. He has a wealth of experience, knowledge and skills to work effectively with multi-disciplinary teams, and families, developing care plans, and co-ordinating complex packages of care, including liaison with and referral to other statutory services across health and social services. Chris has also worked with Health Authority Clinical Commissioning Groups to help facilitate clients’ transfer from rehabilitation settings to independent living. His role also involves the assessment of a client’s functional capabilities and needs before implementing rehabilitation and co-ordinating treatments provided by therapists and other services such as specialist schools.  He is also able to prepare reports, rehabilitation plans and undertake goal setting with clients.

Coming from a social work background Chris has extensive experience of managing challenging and vulnerable people, who have multiple complexities, living within the community and within a residential setting. Academically, Chris holds separate post-graduate master’s qualifications in Addictive Behaviours (MSc) and Advanced Child Protection (MA). He is also qualified in Motivational Interviewing and has also attended the Brain Tree Cognitive Rehabilitation Course.

SINEAD FALLON

Sinead is based in Derbyshire. She holds professional memberships with The British Association of Brain Injury Case Managers (BABICM) as an Advanced Member and Practice Assessor; British Association of Occupational Therapists and The Health Professionals Council.

 Sinead has worked as a Case Manager with HACM Limited since September 2005.  Sinead is an Occupational Therapist and has worked within a variety of specialities within the National Health Service.  Prior to becoming a Case Manager, Sinead was employed by HACM Limited as a Rehabilitation Assistant providing support to clients with traumatic brain injury.  Her role included the implementation and evaluation of rehabilitation programmes within all domains of daily living.

Sinead is now Advanced Associate Case Manager for HACM. She implements and coordinates rehabilitation programmes for adults and children who have sustained an acquired brain injury, Cerebral Palsy, undergone amputation or experienced a traumatic physical, or psychiatric injuries as a result of medical negligence or personal injury. 

Sinead has a wealth of knowledge and skills to enable her to work effectively with multi-disciplinary teams and families by developing care plans, co-ordinating complex packages of care, including liaison with and referral to other statutory services across the NHS and social services as well as therapists in the private sector.  Sinead’s role involves the assessment of a client’s functional capabilities and needs before the onset of rehabilitation and co-ordinating treatment provided by therapists and other services such as schools or employers. Within her case management role, she has experience of working with clients vocationally, helping them return to work by offering advice, support and guidance to individuals who are struggling to manage work requirements alongside their injury or disability.

Sinead has been a board member for BABICM and a committee member for the Children and Young Person’s subgroup. She has been an Advanced BABICM Member since 2010, and more recently an assessor for Advanced Membership applicants. 

Our Office Team

Our Office Team provides support to HACM’s Clients, Case Managers and Care Teams.

Debbie and Melisa typically work behind the scenes to organise and coordinate office administration on a day-to-day basis, to ensure organisational effectiveness and efficiency.  The Office Team are responsible for streamlining administrative procedures, managing databases, finance, and HR.

Having a background in business, finance and operations, Mark Evans acts as Finance Director and Business Manager.

Together, the Office Team handle a wide range of clerical and administrative support related tasks in a timely manner and work as either part of a team or individually, being well organised, flexible and always happy to help.
